Why oh Why

Ok I can understand why photographers want their models to be emaciated because no matter what you do there seems to be a good 20 pounds added to photos.  The problem is though is that this isnt realistic.  I just dont get though how to not add the pounds to pics and end up making someone who is a size 3 look bigger.  Do our eyes betray us or do the lenses make it impossible to look good at the normal sizes.  Even the emaciated models look sickly in photos or like they are on drugs because then they are too thin and gaunt looking.  I have seen very normal looking people that are even a size 7 look good in pics so I wish I knew the secret to taking the pounds off people when I shoot because I seem to be an expert at adding them. :(
